In your own words, describe this SCP. (100 words minimum):
SCP-049 is an entity called "The Plague Doctor" who believes in a disease known as the Pestilence that effected humanity. Although no one else can identify this disease, SCP-049 is completely convinced it exists and dedicates itself to curing it. Its cure is fatal, after which it performs surgery to reanimate the dead as SCP-049-2 instances that obey only it. SCP-049 is highly intelligent, polite, and capable of holding long conversations, especially about medicine and its research. It doesn’t act like a mindless killer, it sees every victim as a patient and believes what it is doing is for the greater good.

How is the SCP maintained?:
SCP-049 is maintained by providing patients or occasionally granting a walk. If a walk is denied, foundation will be needed to provide a patient. During walks, SCP-049 must wear its kevlar gloves and be escorted by three armed personnel. Medical procedures performed alone receive a +15 bonus, and SCP-049 may also provide medical advice to give another person a +15 bonus. While maintenance is taking place and at least three armed personnel are present, SCP-049 cannot breach.
How does this SCP breach? What are their procedures?:
If attacked or left unmaintained, SCP-049 may remove its gloves and breach. Upon breaching, it is immune to FearRP until it successfully creates its first SCP-049-2. It must attempt to create a zombie within one minute and continue actively converting people. SCP-049 may control a maximum of five SCP-049-2 instances at a time, and those instances obey only its commands. SCP-049 may also turn off lights, provided it advertises doing so. If it stops attempting to create zombies after obtaining its first SCP-049-2, it may immediately be Fear rp or terminated.
How does this SCP be recontained?
SCP-049 is recontained by first eliminating all SCP-049-2 instances within its line of sight. Once they have been dealt with, at least three armed personnel must point their weapons at SCP-049 to place it under FearRP. SCP-049 must then put its kevlar gloves back on and be escorted back to its containment chamber. If at any point fewer than three people have their weapons trained on SCP-049 before containment is complete, it may remove its gloves and breach again.

Yes. SCP-049 may be terminated if it intentionally refuses to create SCP-049-2 instances after breaching, they may use any weapons against them
