Name of Test : This doesn't have to be super specific or creative; it just makes it easier for you and us.
Who Approved the Test : Tests can be approved by R&D leads and BRAVO+, but R&D always needs to be informed.
Name and Rank of who performed the test : Self-explanatory; put your name and rank.
Names and Ranks of those involved : Put down the name and rank of anyone present or participating in the test.
Anomaly or SCPs involved (if any) : If the test involved an SCP from Foundation or an undocumented Anomaly, ensure you include that.
Description of Preparation and Intent : Tell us what you did to prepare for the test and why you're doing the test. What are you trying to find, cause, or gain?
Description of outcome and events : Tell us what happened during the test; what happened? How did it end?
Conclusion from same events : What did this test tell you? What did you gain or lose?
Overall success or failure: Do you consider your test a success or a failure?
